TexShare Card Request Form

College faculty and students enrolled in distance learning or traditional classes* can use this form to request a TexShare library card. The TexShare card will allow the cardholder to check out books at participating Texas college, university, and public libraries . You must first have an NHMCCD Library card to request a TexShare Card.

*If you attend any classes on campus at North Harris College, Tomball College, or Cy-Fair College, you may obtain photo ID at the Circulation Desk of your home library. The photo ID is also your library card.

Your TexShare card will be available in a few days and can be picked up at your preferred location or mailed to you.

	TexShare Card Borrower Responsibilities

1. Appropriate identification as required by the lending library must be presented.

2. Materials must be returned BY THE BORROWER TO THE LENDING LIBRARY by the due date shown on the item either in person or by insured mail

3. Materials must be returned immediately if recalled by the lending library.

4. Borrowers are responsible for payment of any fines, damage costs, replacement cost of lost materials, or any other charges assessed by the lending library. Holds will be placed on NHMCCD records until such charges are cleared.

5. Borrowers agree to surrender the TexShare Card to library staff if requested.
